Family Support Worker

The Family Support Worker conducts regular home visits to assess child safety, develop family goals, and monitor progress to prevent out-of-home placements. Responsibilities include connecting families with resources, coordinating with service providers, and facilitating case conferences. Flexible scheduling, including evenings and phone check-ins, is required. The role involves maintaining accurate, confidential documentation in the NYS Connections system, including progress notes and biannual assessments. Participation in supervision, team meetings, and ongoing training is expected. Family Support Workers are mandated reporters. QUALIFICATION AND SKILLS:
