Position Summary
Lead and oversee the hospital’s case management department, ensuring patients receive high-quality, efficient, and coordinated care from admission through discharge and beyond. Develop and implement policies, mentor staff, and collaborate across departments to optimize patient outcomes and hospital performance.
Key Responsibilities
- Direct management and development of the case management team (RN and licensed staff).
- Create and enforce case management protocols, standards, and best practices.
- Monitor patient care coordination, LOS (length of stay), concurrent review, and transitions to prevent readmissions.
- Liaise with medical staff, utilization review, social services, payors, and community providers.
- Track and report on KPIs: utilization metrics, discharge planning efficiency, and compliance.
- Ensure adherence to TJC, CMS, and state regulatory standards.
- Participate in interdisciplinary care planning meetings and discharge rounds.
- Manage department budget, staffing, and training needs.
- Function as Utilization Review/Denials Manager as required.
Qualifications
- Registered Nurse (RN) with a Texas license; relevant certification (CCM, ACM, CCM-RN) preferred.
- Proven leadership experience in acute care, utilization review, case management.
- Strong working knowledge of hospital billing, managed care systems, and discharge planning.
- Excellent skills in communication, collaboration, decision-making, and problem-solving.
- Ability to interpret utilization data and apply quality improvement methodologies.
- Familiarity with electronic health record (PACS, health IT systems) preferred.
