Community Family Navigation Specialist
Salary Grade: Grade 16, Starting Salary: $43,676 + Competitive Benefits Package
Position Summary: The Community Family Navigation Specialist will serve as a trusted liaison between the Iredell County Partnership for Young Children, partner agencies, families, and community resources. The Community Family Navigation Specialist will implement the Family Navigation model using the Colorado Family Support Assessment 2.0 (CFSA 2.0). Embedded on-site at partner agencies, this role is responsible for building relationships with families, identifying needs, and connecting them to services that support stability, health, and well-being. The Community Family Navigation Specialist will work collaboratively with partner agency staff and external partners to ensure families have access to wraparound support. Essential Duties and Responsibilities: Community Engagement • Build trusting relationships with families of partner agencies through regular presence and engagement. • Participate in community meetings, training, and collaborative initiatives. Family Navigation • Schedule appointments with families to conduct an initial assessment. • Assess family strengths and needs utilizing CFSA 2.0. • Assess protective factors using Protective Factors Survey. • Assess readiness for change and assist family with goal setting. • Utilize family support goal setting techniques for decision making and problem solving. • Identify resources available to families and coordinate with agencies as needed to access services. • Refer family to community resources based on identified needs and help with acquisition. • Schedule follow up assessments at appropriate intervals to monitor progress towards family self-reliance and track protective factors that reduce the likelihood of child abuse and neglect. • Schedule family support meetings to foster continued family engagement between assessments. • Distribute surveys to Family Navigation participants. • Advocate for families and help them navigate complex systems (e.g., social services, schools, healthcare). • Promote a strength-based, culturally responsive approach to family engagement. Recordkeeping/Reporting • Maintain accurate and confidential records of family interactions and referrals. • Track outcomes and report on service utilization and impact. • Complete reporting records as requested.
Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily and meet the requirements listed below. • Must pass a criminal background check. • Must have reliable transportation and valid driver’s license. • Occasional evenings, early mornings, and/or weekend hours required. • Local travel is required. Education and/or Experience: Bachelor’s degree in a field related to family studies, child development, or human services from an accredited college or university and four (4) or more years’ related experience providing direct service to families required. Equivalent combination of education and experience considered. Prior experience administering the Colorado Family Support Assessment 2.0 (CFSA 2.0) is not required. Training will be provided. Work Environment: This position is a part of the Whole 3 (Whole Child, Whole Family, Whole Community) Collaborative* and will be embedded at partner agencies. Schedule to be determined. Occasional evening or weekend hours may be required to meet family needs or attend events. Knowledge and Abilities: • Must be able to connect with families, children, and coworkers with enthusiasm and professionalism. • Knowledge of Standards of Quality for Family Strengthening & Support and Strengthening Families and the Protective Factors Framework (Training will be provided). • Knowledge of cultural responsiveness, cultural humility, and motivational interviewing (Training will be provided). • Proficiency in Microsoft Applications. • Ability to communicate and work effectively with people from diverse backgrounds. • Ability to adapt to and support a changing work environment and new demands. • Effective oral/written communication skills. • Demonstration of proper use of English language, business composition, spelling and punctuation. • Ability to work independently and as part of an integrated team. • A high level of organizational and interpersonal skills. • Ability to lift up to 40 pounds. * This grant-funded position is a limited-term appointment, anticipated to last for three years, contingent upon grant funding availability
