COMMUNITY HEALTH REPRESENTATIVE Polson TRIBAL HEALTH DEPARTMENT The successful applicant must have a minimum of 2-yearsexperience working in the health care setting OR Proof of aCertified Nursing Assistant course completion with a minimumof 1-year work as a nursing assistant. Valid and unrestricteddrivers license. Heart saver CPR or BLS within six months ofhire. Experience with an electronic health record preferred.Must pass a background and suitability check according to Public Law 101-630; the Indian Child Protection and Family Violence Prevention Act. All applicants must submit a Tribalapplication, a copy of academic transcript, certifications, proofof state licensure, copy of valid drivers license, proof ofenrollment from a federally recognized Tribe if other than CSKT,and if claiming veterans preference, a copy of the DD214 mustbe submitted. FAILURE TO SUBMIT ALL OF THE ABOVEINFORMATION WILL RESULT IN DISQUALIFICATIONDURING THE SCREENING PROCESS. This position is aTesting Designated Position (TDP) within the definition ofthe CSKT Drug Testing policy. The successful applicant,if not already employed by the Tribes must pass a pre-hire drug test and serve a mandatory six (6) month probationary period.
