MH Director of Throughput and Patient Flow

Summary:

The Director of Throughput and Patient Flow will report to the Senior Director of Nursing and CNO. Provides comprehensive direction translates and supports the goals and objectives policies and procedures and ensures high standards of care for patients at Morton Hospital. In collaboration with Nursing and Hospital Leadership the Director of Throughput and Patient Flow is responsible for removing barriers to ensure timely throughput across all areas of the Hospital. Will participate in interdisciplinary rounds discharge rounds and staffing huddle. Will serve as the co-chair of the hospital*s Throughput Committee.

Responsibilities:

  • Coordinates patient flow with nurses doctors hospital administrators and other healthcare professionals

  • Ensure patients move easily through and within the healthcare system in a timely organized and efficient manner

  • Promotes quality cost effective management of patient care reviewing concurrently utilization of drug formulary etc.

  • Works with the Epicenter team to facilitate load balancing when necessary.

  • Collects statistics for redraw on conversion rates LOS test utilization.

  • Contribute to the hospital discharge process by identifying patients with lengthy facility stays and collaborating with the multidisciplinary team to expedite the process

  • Works as part of a team to achieve the patients* daily goals and coordinates care to achieve a timely discharge

  • Familiar with observation criteria and collaborates with case management on conversations for in-patient status

  • Identify areas that cause ineffectiveness and/or delays in a timely discharge

  • Work with the emergency department to monitor and evaluate the need for functional and accessible emergency beds for patients that may come from the emergency room

  • Maintain and hasten the transfer of admitted patients from the emergency department to other departments to improve the flow of patients within the facility

  • Co-chairs the Patient Throughput Committee

  • Oversees various personnel actions including but not limited to hiring performance appraisals disciplinary actions and other related activities.

  • Facilitate and maintain good communication with other hospitals to manage escalations efficiently amid busy periods and manage patients from emergency care

  • Metric Responsibilities * Patient Length of Stay versus Geometric Mean Length of Stay Discharge Order Written Early Discharge Order to Patient Departure Average Discharge Time Per Unit ED Throughput Metrics * Decision to Admit to Arrival on Unit.

Performs other duties as assigned.

    EDUCATION:

    Bachelors of Nursing

    ACLS BLS certifications or receives within 3 months of hire

    Preferred Qualifications:

    Masters of Nursing

    EXPERIENCE:

    5-8 years of crucial experience telemetry house supervision

    SUPERVISORY RESPONSIBILITY:

    Direct Supervises up to 50 FTEs including hiring orientation training and generation of personneel actions or recommenations.

    * Patient when responding to questions and answers questions appropriately

    * Attention to detail

    * Responds to requestsin a timely manner

    * Handles interruptions in a skillful way

    * Strives to prevent/resolve customer concerns to the customer*s satisfaction

    * Assigns staff to patients knowing the competencies of all staff involved

    * Responds and provides service recovery within 24 hours

    REQUIRED KNOWLEDGE & SKILLS:

    • Is responsible for ongoing development of his/her work skills through the useof availableresources (i.e. in-services formal educational programs other work groups and on-the-job training).

    • Attends andparticipatesin staff meetings and in-services and/or reviews documentation asrequired

    • Assistsin the orientation of others and activelyparticipatesin mentoring

    • Performs analytical anddecision makingfunctions with minimal supervision

    • Recognizes and seeks assistance/consultation when appropriate

    • Demonstratesan understanding of relative Hospital and all-departmental policies and procedures including safety issues

    • Strong knowledge of Microsoft products * Excel Word PowerPoint etc.

    • Possessawareness of programs/services provided by Hospital and where they are located
