Responsibilities

  • Performs home visits, phone consultations or telehealth to patients with nutritional problems as ordered by the physician.

  • Assesses patients who may have:

  1. Difficulty in complying or understanding dietary restrictions.

  2. Moderate to high nutritional risk.
  3. Evidence of malnutrition, slow wound healing process or significant weight gain or loss.
  4. Complications related to long term, peg-tube feedings.
  • Assists in teaching and counseling the patient, family and other caregivers' on the specific dietary regimen.

  • Evaluates the nutritional needs of the patient and assists in planning for home dietary management for nutrients, texture, food allergies and food intolerance.

  • Records observations and activities related to patient care. Submits documentation.
  • Provides in-service to other home health personnel at least annually, when requested.
  • Evaluates and assists in managing the lifestyle factors affecting the patient's diagnosis such as diabetes and chronic kidney disease.
  • Participates in the management of tube feedings.

Qualifications

  • Registered by the Commission on Dietetic Registration of the American Dietetic Association.

  • Current dietitian license, specific to that state you are assigned to work in.
  • Five (5+) years of professional experience as a registered dietician.
  • Current CPR certification.
