Clinical Social Worker, Care Management - North Shore

As a Clinical Social Worker, you will play a vital role in identifying and addressing high-risk psychosocial factors, helping primary care teams understand their impact, and providing comprehensive support and intervention. Depending on each case, you will either deliver treatment directly or coordinate with outpatient clinicians to ensure effective, patient-centered care.
Key Responsibilities:
- Comprehensive Assessment & Case Management - Identify patient needs and develop tailored care plans.
-Patient Navigation & Advocacy - Guide patients through healthcare systems and connect them with essential resources.
-Health Promotion & Self-Management Education - Empower patients to take charge of their well-being.
-Crisis Intervention & Transitional Care - Provide urgent support and smooth transitions between levels of care.
-Short-Term Counseling & Advanced Care Planning - Offer emotional and logistical support to patients and families.
-Collaboration & Consultation - Work closely with physicians, nurses, and behavioral health specialists to enhance patient outcomes.

Who We're Looking For:

  • Licensed Clinical Social Worker (LICSW/LCSW) in Massachusetts
  • Master's degree in Social Work
  • Minimum 2 years of clinical experience, preferably in care coordination, case management, or post-acute care
  • Strong communication and interpersonal skills to build trust with patients and healthcare teams
  • Ability to work both independently and collaboratively in a fast-paced environment
