Position Title: Social Service Case Manager

Report To: Social Services Ministry Director

Pay Grade: 205

Pay Range: $19.97 to $23.96 Hourly

Qualifications:

Education: Minimum of a Bachelor's degree in human services or related field. Equivalent combination of education and experience will be considered.

Experience: Outgoing personality with good communication skills and ability to work with diverse cultures are necessary. Strong leadership style and good organizational skills required. Knowledge of community resources including federal, state and municipal government and requirements of each resource is required. Ability to pass background check, pass and maintain TSA approved driving status (MVR). Ability to demonstrate professionalism in relationships within the organization and in the larger community. Must be proficient in Microsoft Word, Excel and the internet. Ability to support and transmit the mission of The Salvation Army.

Responsibilities:



  • Implement programs to ensure delivery of services and compliance with all funder requirements. This includes, but is not limited to, the following tasks:
  • Provide or coordinate case management and develop comprehensive individualized service plans and goals for each program participant.
  • Maintain ongoing contact with program participants.
  • Keep extensive daily documentation as required by The Salvation Army and other agencies.
  • Maintain client confidentiality.
  • Regularly facilitate meetings and workshops.
  • Establish and maintain relationships with area social service providers by attending meetings, conferences, and councils. Make referrals for program participants as needed.
  • Demonstrate ability to learn ServicePoint program to enter data into HMIS system. Demonstrate ability to develop and create a spreadsheet tracking system, to enter data, or to train and oversee staff to perform that function. Ensure data is complete and accurate.
  • Assist in the preparation of departmental budgets. Monitor and control spending to comply with departmental budgets in collaboration with the Business Administrator.
  • Collect data, compile, and submit monthly reports on all department activity for use of Advisory Board and to submit for monthly statistics.
  • Provide input to Grants Management Team on existing, renewal and potential new grants related to homeless services programs. In this context, assume responsibility for timely implementation and reporting for existing grants in collaboration with the Business Administrator.


  • Maintain positive work atmosphere by behaving and communicating in a professional manner that fosters good relationships with program participants, coworkers and supervisors.
  • Perform any other duties as assigned by Corps Officers.

Regular, Full Time, Non-Exempt

Physical Demands/ Work Environment:

This position requires light to moderate physical activity. In order to successfully perform the essential functions of this position, the individual is regularly required to lift and move 30 lbs , remain stationary for some periods of time, use hands and fingers, communicate, move, occasionally required to position self to stoop, kneel or crouch, frequently required to reach with and use arms and hands as required in using standard office equipment and a personal computer. Specific vision abilities by this job include close vision, distance vision and ability to focus. The work environment for this position includes an office environment with a medium/high noise level due to conversations in the area.
