Overview
The Intake Coordinator is primarily responsible for answering all inquiries and following up in a timely manner. Must have excellent customer service skills, be a team player, and use proper phone etiquette at all times. Responsible for taking control of every inquiry and scheduling care plan visits for patients across the life span (perinatal, pediatrics, and adults) in accordance with the department and agency expectations. The Elizabeth Hospice is San Diego's oldest non-profit hospice of 45 years. We are growing and looking for talented individuals to join our high performing team and support our mission. We serve all of San Diego county and south Riverside county with multiple hospice office locations. Our Mission: To enhance the quality of life for those nearing the end of life's journey and for those who grieve. Salary: $25 - $27 hourly. Subject to experience and qualifications. Work Schedule: Monday, Tuesday, Wednesday, Friday,Saturday 10:00am - 7:00pm Work Location: Escondido, CA Travel: Infrequent for position, if at all.
Responsibilities
Maintains professional relationships with referral sources (hospitals, skilled nursing, assisted living facilities, RCFEs, physician associations and independent medical clinics/offices) and meets the needs of every customer.
- Explain hospice benefits and services to patients and families as needed.
- Process new referrals by telephone, email, fax, and web-based portals.
- Document all inquiries in patient database.
- Schedule all care plan visits and record in scheduler.
- Acquire all H&P and data material on admission referrals and record information accurately.
- Contact physicians, patients, families, and case managers to schedule an admission presentation.
- Verify Medicare and Medicaid hospice benefits.
- Schedule and convey necessary information to admissions nurses, SW's, and managers.
- Responsible for daily routine follow on pending charts.
- Portray self-confidence with the agency's reputation and translates this image into a trusting relationship both with patients/families and customers.
- Assist with implementation and coordination of services for patients discharging from facility to begin service with The Elizabeth Hospice.
- Other job duties and special projects as assigned.
Qualifications
- High school diploma or equivalent and a minimum experience of 2 or more years in healthcare or call center environment.
- Excellent verbal and written communication skills.
- Ability to answer phones, maintains patient referral records, and schedule appointments.
- Strong organizational skills and manage multiple responsibilities simultaneously.
- Bilingual - Spanish speaking preferred.
- Demonstrated ability to work closely with a variety of people while effectively using problem solving skills.
- Demonstrated commitment to hospice philosophy of care.
- Proficiency in office software applications and customized call center applications, required.
- Ability to work weekends and holidays.
- Strong attention to detail
- Ability to work in a results-oriented team environment.
- Current California driver's license and proof of auto insurance.
