Position Summary |
The
UNC Formerly Incarcerated Transition (
FIT) Program is a partnership between
UNC Family Medicine, the North Carolina Department of Adult Correction (which administers the state prisons), Federally Qualified Health Centers, County Departments of Health, and local reentry councils. Upon release from the state prison system, individuals are frequently without work, without health insurance, and without a medical home. The
FIT Program aims to connect all formerly incarcerated individuals who have a chronic disease with a primary care provider. The program hinges on Community Health Workers (
CHW) making contact with individuals who are incarcerated with chronic disease prior to release, developing rapport, and helping put together a comprehensive reentry plan. Since its inception,
FIT has taken on additional projects that align with the program's mission to improve health outcomes and decrease recidivism for formerly incarcerated people.
In 2022, the
FIT Program launched a pilot, named
FIT Wellness, to provide psychiatric and primary care services to individuals with serious mental illness (
SMI) releasing from prison in Wake County. Given the success of the pilot, the state has awarded the program with funding to continue the work in Wake County and expand services to Orange, Durham, and New Hanover counties. The
UNC Department of Family Medicine seeks to hire a full-time Program Coordinator to help advance the Program's goals of reducing recidivism, improving health outcomes, and supporting reintegration. The
FIT Wellness Program Coordinator will help coordinate the daily operations of the Program, including providing essential administrative, financial, programmatic, and research support, communicating with key community members, and assisting with expansion activities. This position will work closely with the
FIT Wellness Program Manager, Medical Director, and community health workers on a highly interdisciplinary team. |
Minimum Education and Experience Requirements |
Bachelor's degree in business administration or a related discipline; or an equivalent combination of training and experience. All degrees must be received from appropriately accredited institutions. |
Required Qualifications, Competencies, and Experience |
* Must have experience with project management project startups, or project implementation.
* Must have experience communicating with diverse audiences and building relationships with a variety of partners and constituents.
* Candidate must be able to demonstrate the capacity to manage multiple responsibilities of a confidential nature and exercise a high level of discretion and independent judgment and ability to work autonomously
* Must demonstrate strong time management skills and be able to prioritize timeliness in responding to internal and external partners.
* Candidate must be able to demonstrate excellent written and oral communication, and a strong attention to detail.
* Candidate must have proven administrative abilities and an understanding of the culture, policies, and procedures relevant to academic or community health programs.
* Must be proficient in Microsoft Office programs. |
Preferred Qualifications, Competencies, and Experience |
* Knowledge of Public Health field and work experience in the field of public health management.
* Knowledge and experience in health issues related to criminal legal system involvement highly preferred.
* Experience working with individuals with serious mental illnesses.
* Experience with client assessment, treatment planning, conducting interviews/investigations, advising/counseling clients, and participating in health promotion and health education activities.
* Experience with grant management, budgeting, and continuous improvement.
* Skill in coordinating workflows, managing multiple priorities, exercising independent judgment, and communicating with diverse audiences.
* Proficiency in administrative tasks such as data management, scheduling, maintaining accurate records, and report generation.
* Familiarity with Epic and Qualtrics systems.
* Ability to draft professional written materials, communicate program information, and provide excellent verbal support to team members and stakeholders. |
