Minimum to Midpoint Pay Rate:$30.27 - $40.86 / hour

The Post-Acute Care Manager is responsible for partnering with other members of the acute/post-acute healthcare team to insure potential referrals are proactively contacted, triaged and assessed for admission. Conducts and documents a thorough clinical, social, functional and financial assessment consistently utilizing criteria for admission and identifying the reason for acute inpatient rehabilitation, skilled nursing facility, skilled nursing unit, or home health. Discusses options available with the patient and/or designated representative to assure informed decision-making and consent occurs. Conducts strategic and focused contacts with key stakeholders to assure consistent education regarding admission criteria, continued census development and authorization for admission is appropriately obtained. Attends clinical rounds, discusses patient plan of care with care management to transition patients across the appropriate post-acute setting.

Previous experience in discharge planning, assessment skills and clinical social factors that affect patients functional status at discharge preferred.

Previous Experience in Home Health, or Inpatient Rehabilitation Nursing, or Skilled Nursing Facility/Unit preferred

Case Management or Discharge Planning is required
